You can request the appropriate requisite booking forms required to complete your event booking by emailing us.

Once you have obtained a form, please return the completed form to the booking office at Leamington Spa Gurdwara. Our booking officers will then review the form to ensure all required fields are completed, and requested dates are available.

NOTE: Only once the forms have been completed, signed off and payment received will the booking be confirmed.
